Request Health Records

To request health records, follow the steps below.

To view records online through Health Gateway, please ask your BC Cancer health care team to copy you on their reports. Please note that not all records are available through Health Gateway and must be requested.

If you are requesting your own health records, you can submit the request using this online form.

Please note if you are requesting someone else’s health records, you must complete and submit the form below – you can also use this form to request your own health records if you do not wish to use the online form:

Email, fax, mail or drop off the completed form to the Health Information Management Release of Information Office of the centre or hospital from which you are requesting records. See the contact list for email addresses, addresses, phone and fax numbers
